What do I do if it hurts when my sister cries?

What you describe sounds like a manifestation of autonomic dysfunction.
I think you do not really need to be indifferent to children. You need to try to listen to yourself and think about what memories you have of a baby crying. Talk to your parents about it. Perhaps there were circumstances when you were not given enough attention as a child. Maybe there is an untreated early deprivation - you were sent to kindergarten early and the teachers were cold. If you can talk to the parents about it, do it. There will probably be tears, and then it will get easier.
